Title Case Manager - Emergency Department

Description Acts as a resource in the Emergency Department (ED) for providers to determine correct admission status utilizing intensity of service and severity of illness criteria to maintain appropriate care level and utilization of hospitalization outside of normal business hours. Interacts with and manages high risk, complex, resource intensive, and frequent ED patients in order to optimize clinical, functional, and cost effective outcomes. Provides education to patient and family about available resources. Coordinates with care team to ensure communication of transition plans. Collaborates with ED nurse/ED charge nurse to establish discharge planning needs consistent with the overall outcomes/goals and regulatory guidelines and assists with the discharge planning process for those patients going home with identified resources. Communicates with community resources. Coordinates with care team to ensure appropriate services are in place to avoid inappropriate admissions and prevent hospital readmissions when possible.
Requirements Registered Nursing degree licensed in the State of Ohio with at least 2 years of acute care or case management experience, BSN preferred; OR Licensed Independent Social Worker with at least 2 years of acute care or case management or mental health experience. OR Licensed Social Worker with at least 3 years of acute care or case management or mental health experience. Excellent communication, customer service, and computer skills required. Maintains continuous certification in American Heart Association’s BLS. Must complete Memorial Behavioral Health and De-escalation Class.
