Description The Treatment Coordinator is responsible for the coordination of clinical services to all individuals receiving services from the Article 16 Health Center. Participates in treatment/program planning and maintains appropriate records, statistical reports, etc. so as to ensure appropriate treatment activity as well as maintain program efficacy.

ESSENTIAL FUNCTIONS/RESPONSIBILITIES

•Review intake and admission procedures and ensure that the required paper work is completed (check all referral materials as per referral guidelines).
•Obtain any necessary and past information from the referral source and/or present programs outside of the EHC and ensure that there is no duplication of service (ensure that all individuals’ records have the “Information about Current Clinical Services” form)
•Counsel the service recipient and/or collateral as to the EHC’s procedures, rights of consumers etc.
•Bring any concerns to the attention of the appropriate Clinic personnel and follow up to ensure resolution
•Ensure that the individual is receiving the service(s) as scheduled / recommended (verify the same with clinician time log and contact notes).
•Follow up with the service providers as to the current status of treatment (review the progress as well as modifications of the treatment plan during the semiannual and annual review meetings).
•Transmit required information to referral sources, the service coordinator and/or other interested appropriate parties (ensure that ISP’s, and other relevant treatment team documents list EHC as the provider of clinical services including the frequency of service delivery).
•Ensure that the contact notes are up to date in the EHC record (review contact notes for quality on a monthly basis).
•Follow up on outside referrals by the EHC for ancillary services and ensure that any resulting information is entered into the record in a timely manner.
•Evaluate the individual’s / collateral’s satisfaction with the service(s) (maintains phone logs as well as service satisfaction surveys in order to ensure satisfaction of the service delivery process).
•Review the clinical record and provide explanations for its content (conduct record reviews using the file review form).
•Ensure that the annual physician reviews, and the annual review of services that the Treatment Coordinator completes continue to document the need for the service(s) provided.
•Conduct semiannual and annual reviews to integrate all services that the consumer is receiving through the Article 16 Elmwood Health Center.
•Participate in Quality Assurance Activities.

•Supports EHC and People Inc. by accepting additional tasks outside of job description such as special projects, public education, etc.
•Plans time off in advance and manages PTO according to department guidelines.


SUPERVISORY DUTIES:

•While this position has no direct supervisory responsibility, the Treatment Coordinator will provide guidance to a multidisciplinary team responsible to provide services to the Developmentally Disabled.
Requirements •Bachelor’s Degree in Nursing, Social Work or Education required; Master’s preferred.
•2+ years experience working with the Developmentally Disabled.
