Description Provides physical therapy services under the supervision of designated licensed physical therapists. Completes individual treatments on maintenance/restorative patients at the clinic with on site supervision. Completes individual treatments on long-term/maintenance patients at day treatments and ICF’s, and at the clinic. Supervision of a Physical therapy assistant by a licensed physical therapist shall be continuous but not necessarily on site when the supervising physical therapist has determined, through evaluation, the setting of goals and the establishment of a treatment plan, which the program is of maintenance. Provides training to staff on patient treatment plans, protocols, including positioning, transfers and ambulation. Follows guidelines set forth by the quality management committee. Participates in patient and staff education and other duties as required. Receives professional supervision as required by Education Law, Article 136, section 6738.

ESSENTIAL FUNCTIONS/RESPONSIBILITIES

•Provides direct therapy services and documents treatments as required.
•Implements treatment plans for those individuals recommended for therapy in conjunction with physician approval. Monitors for changes needed in treatment plans and reports to supervising physical therapist.
•As a member of the Interdisciplinary treatment Team, participates in professional meetings as assigned, integrating services with other providers.
•Completes dayhab and clinic documentation in accordance with policies and regulations, including completing subjective/objective data for treatment plans/semi-annuals, and reviewing assessment, goals, and plan with supervising PT.
•Makes recommendations for the purchase of specialized equipment, materials, and wheelchair modifications.
•Counsel patients and their family members/care givers (if applicable) regarding the patient’s Physical Therapy Services including staff/family education upon discharge.
•As a member of the Interdisciplinary Treatment Team, participates in professional meetings as assigned.
•Utilizes independent judgment to develop and revise written plans/ staff actions/ protocols for staff to implement.
•Provides training to staff on patient treatment plans, protocols, including positioning, transfers and ambulation.
•Participates in in-service and agency training as needed.
•Provides supervision to staff assisting with therapy programs.
•Assists in provision of therapy services in ancillary clinics as necessary.
•Oversees patients in the Wellness Program at the Clinic as needed.
•Maintain productivity expectations at minimum of 1.35/patients/ hour.
•Functions as a clinical instructor for PTA students yearly (with minimum 1 yr work experience).
•Maintain clinical competency by attending/completing at least one continuing education class/course each year.
•Maintain 90% compliance on Chart Audits and follows required documentation guidelines/expectations set forth by the department and the Insurance/Medicare/ Medicaid.
•Maintains SCIP-R Core Certification.
•Complies with all agency policies and procedure.
•Other duties as assigned.

•Plans time off in advance and manages PTO according to department guidelines.
Requirements •Graduate of an accredited program for Physical Therapist Assistants.
•Eligible for certification or certified and currently registered to practice as a Physical Therapy Assistant in New York State. For new graduates, must be completed within 6 months of employment.
•Meet requirements for continuing education in accordance with the New York State Board of Education.
•Lifting requirement of 50 lbs.
•Physical agility and ability to react to emergency situations, including maintaining Strategies for Crisis Intervention and Prevention Core Training certification.
•CPR certification.
