LEVEL/FLSA: 14, non-exempt

** This position will support Orthopedics, General Surgery, Breast Surgery, and Urology **

GENERAL SUMMARY: The Patient Care Coordinator focuses on population management with demonstrated capability to provide relationship-based care within a collaborative health care team. This nurse utilizes advanced critical thinking to develop, implement and evaluate an individualized plan of care that educates, empowers and optimizes patient outcomes throughout all transitions within the continuum of care.

PRINCIPAL DUTIES AND RESPONSIBILITIES:
Essential Functions of the Job:
 In collaboration with provider, performs a focused assessment to evaluate patients and their support systems for real and potential health and safety issues, educational needs, and resource availability upon initial interaction, throughout care and/or treatment, and at times of transition.
 In collaboration with provider, contributes to the development of the plan of care.
 Monitor, trend, and record patient’s responses to disease, treatment and preventative strategies.
 Explore patient’s understanding and knowledge of current health care status and provide education.
 Investigate and utilize information beyond the medical conditions to include patient values, preferences, literacy, social context and cultural dimensions.
 Collaborate with patient and support system in establishing mutual goals based upon patient’s needs and preferences.
 Provide education to patients in preparation for procedures and testing.
 Room patient and complete intake process per protocols.
 Administer medications and vaccines. Perform venipuncture and ambulatory procedures within scope of practice.
 Laboratory duties including performing point of care testing (POCT) and quality control indicators, preparing forms and follow up results.
 Assist provider with test result tracking, communication and correspondence to patients.
 Assist providers in procedures and performs various test independently.
 Use clinical equipment for testing as ordered.
 Evaluate lab work and advise provider of critical values.
 Manage EMR requirements to include: in basket, phone calls, prescriptions and results in a timely manner.
 Answer telephones, confirm appointments, maintain appointment schedule following office scheduling procedures.
 Obtain prior authorizations, waivers and consent for medications and procedures as needed.
 Triage patients and advise per protocol.
 Maintain appropriate inventory of medical supplies for the practice.
 Maintain medical instruments and ensure clinical areas are appropriately stocked and cleaned.
 Document and charge capture for service rendered in Medical Record.
 Maintains appropriate inventory of pharmaceutical, lab and medical supplies for the practice. Works with Office Manager to monitor budget and stay within budget before ordering supplies.
 May serve as a back-up to Office Manager or Office Supervisor.
 Other duties that may be specific to satellite medical practice.
Non-Essential Functions of the Job:
 Provides administrative support as needed.

Requirements REQUIRED QUALIFICATIONS (Knowledge, Skills & Abilities):
Education:
 High School Diploma or GED
 Current Virginia RN license
Experience:
 Minimum of 3 years related experience preferred
 Must have experience operating lab and medical equipment
 Knowledge of OSHA guidelines and Universal Precautions protocols
Knowledge and skills:
 Experience utilizing a personal computer and computerized scheduling
