Description POSITION TITLE: Certified Nursing Assistant (CNA)
JOB CODE: C2022 LEVEL/FLSA: 7, Non-exempt
REPORTS TO: Advance Practice Provider (APP)

GENERAL SUMMARY:
The Certified Nursing Assistant (CNA) performs a variety of patient care and administrative support functions, reports patient protocol responses under the supervision of the Family Advance Practice Provider in an outpatient setting or in a patient’s home.

*This position is anticipated to be a 3 year program*

PRINCIPAL DUTIES AND RESPONSIBILITIES:

1. Assists professional healthcare staff with performing physical examinations, which may include measuring and recording vital signs and recording/reporting patient responses to established protocols.
a. Patient data, such as vital signs and weights, are taken and recorded according to policy and procedure.
b. Changes and abnormal findings in patient’s data are communicated to the Advance Practice Provider (APP) supervisor and other members of the healthcare team in a timely manner.
c. Reinforces selected patient teaching initiated by the APP.
d. Contributes to the development of the patient plan of care under the direction of APP.
e. Performs home visits for home safety assessments, medication compliance, reinforcement of teaching and protocol administration
f. Completes required patient documentation accurately and in a timely manner.

2. Maintains patient safety.
a. Patient’s environment is kept neat and clean.
b. Uses infection control measures to prevent the spread of infection in the home, between family members and for self-protection.
c. Equipment maintenance and safety checks are completed according to policy and procedure.
d. Incidents are reported promptly to appropriate parties using the Health System’s quality reporting process.
e. Reinforces medication regimen with patient and caregivers.

3. Performs administrative support functions.
a. Medical record duties, including file maintenance and record keeping, are completed when necessary.
b. Supply inventory and ordering are completed according to guidelines.
c. Duties, including scheduling home visits, greeting patients, and completing administration of home monitoring protocols are performed efficiently.

4. Maintains necessary skills and competencies.
a. Competency in the use of new equipment (i.e., lifting and moving patients) is achieved and maintained.
b. Strengths and opportunities for professional development are identified and goals for self-improvement are set and documented appropriately.
c. Identified goals for professional development are met through a variety of educational forums.
d. The education and development of others is fostered by sharing information learned through individual professional development.
e. A positive environment conducive to professional development of coworkers is demonstrated on an ongoing basis including but not limited to teaching, orienting, role modeling and team participation.
f. Annual mandatory training activities and regulatory in-service hours requirements are completed with-in established timeframes.

Requirements REQUIRED QUALIFICATIONS (Knowledge, Skills & Abilities):
Education:
 High School Diploma or GED.
 License/Certification: Must have a current Certified Nursing Assistant certification (CNA) through the Virginia Board of Nursing, or verification from the VA Board of Nursing that their existing (CNA) from another state is valid and they are applying for a VA CNA. BLS certification required.

Skills:
 Basic knowledge of and experience utilizing a personal computer

WORKING CONDITIONS: Job requires frequently bending/stooping and driving. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly. May be exposed to chemicals, blood/body fluids and infectious disease.
